Requirements

Practicum requires a minimum of 45 clock hours to be completed during the
semester
. All placements are assigned and confirmed through the Field Placement Office.

Practicum Placement Form

A form must be completed for each practicum. Please complete and submit the form on line.

Required Clearances

TB Test must be valid throughout practicum term. TB Test may be obtained from current employer,
private physician, local health clinic, county health dept. or at Dearborn Public Health Department. An
official form with dates/times of testing and verification of payment by UM-D can be obtained from the Field
Placement Office 261CS. The TB clearance is valid for three years.

Criminal Background Check (CBC) please check with your local law enforcement agency to determine
whether they are offering livescan fingerprinting or you may contact the state approved vendor, Integrated Biometric Technology. Fingerprinting/criminal background check is also required for field placement and must be obtained prior to practicum assignment. The background check is valid for and should be valid until the end of the practicum semester. Visit the Field Placement Office261 FCS if you need forms, have questions or need further information (313 593-5094) forms are available in 262 FCS.

The Bloodborne Pathogens and Infectious Diseases Video must be viewed and the accompanying
test must be passed. Video may be viewed either in the Curriculum Lab, room 267, (313-593-5329) or the
Educational Psychology EDC 300 or Child Development EDC 340 classes.
